Best way to segment patients for a new telehealth app?

Trying to figure out the right segmentation approach — demographics feel too shallow but we're not sure what else to anchor on. Open to any ideas.

Demographics alone are definitely too shallow for telehealth — I'd anchor on health behavior and tech comfort level instead. Think about segmenting by things like chronic vs. acute care needs, self-management motivation, and prior digital health experience. A quick cluster analysis on even 5-6 behavioral variables usually surfaces 3-4 distinct personas that actually drive product decisions.
