Use neutral anchor scales like 'How would you describe your experience?' instead of 'How satisfied were you?' — that alone cuts a lot of bias. Also, mix in a few open-ended questions so patients can surface issues you didn't anticipate. Tools like Qualtrics have built-in survey bias checkers that are worth running before you launch.
