Partner with patient advocacy groups or chronic pain communities on Facebook/Reddit — people there are already talking about their experiences and tend to be way more willing to participate. Make it clear upfront that it's anonymous and takes under 10 minutes, that alone drops the 'invasive' feeling a lot. A small incentive like a $10 Amazon gift card also goes a long way without feeling transactional.
