Swap clinical language for first-person phrasing — instead of 'Rate your pain level,' try 'How much did pain get in the way of your day?' Also, tools like Typeform let you add branching logic so it feels more like a chat than a form. Pilot it with 5 real users first; they'll flag anything that sounds cold faster than any internal review will.
