Best way to survey patients without making it feel too clinical?

Working on a patient experience study and worried the survey language is too formal and putting people off. Anyone dealt with this before?

Totally dealt with this — swap out clinical terms for conversational ones (e.g. 'How did your visit go?' instead of 'Rate your patient experience'). Tools like Typeform help a lot because the one-question-at-a-time format feels more like a chat than a form. Also keeping it under 10 questions makes a huge difference in completion rates.
